Hybrid Remote Case Manager - Skilled Nursing Facilities

Location: New York, NY (Hybrid Remote: 2 days work from home, 3 days onsite assessments at skilled nursing facilities)

Position Type: Full-time

Job Summary: We are seeking a motivated and experienced Registered Nurse (RN) to join our team as a Hybrid Remote Case Manager specializing in Skilled Nursing Facilities. In this role, you will be responsible for conducting assessments and providing case management services for patients in skilled nursing facilities, with a hybrid work model consisting of two days of remote work and three days of onsite assessments. The ideal candidate will have a valid New York RN license and a strong background in case management, care coordination, and clinical nursing.

Key Responsibilities:

  • Conduct comprehensive assessments of patients in skilled nursing facilities to evaluate their medical, psychosocial, and functional needs.
  • Develop individualized care plans based on assessment findings, in collaboration with the interdisciplinary healthcare team and the patient's primary care provider.
  • Coordinate and facilitate the delivery of healthcare services, including medical treatments, therapies, medications, and social services, to ensure continuity and quality of care.
  • Monitor patient progress and outcomes, and adjust care plans as necessary to meet evolving needs and goals.
  • Provide education and support to patients and their families regarding health conditions, treatment options, self-management strategies, and community resources.
  • Collaborate with facility staff, community agencies, and other healthcare providers to optimize care transitions and promote seamless transitions of care.
  • Document assessments, care plans, interventions, and outcomes accurately and timely in the electronic health record system.
  • Participate in interdisciplinary team meetings, case conferences, and quality improvement initiatives to enhance patient care and outcomes.

Qualifications:

  • Valid Registered Nurse (RN) license in the state of New York.
  • Bachelor's degree in Nursing (BSN) preferred.
  • Minimum of 2 years of clinical nursing experience, with a focus on geriatrics, long-term care, or rehabilitation.
  • Previous experience in case management, care coordination, or utilization review preferred.
  • Strong assessment, critical thinking, and problem-solving skills.
  • Excellent communication and interpersonal skills, with the ability to effectively engage patients, families, and healthcare providers.
  • Proficiency in using electronic health record systems and other healthcare technology.
  • Ability to work independently and as part of a collaborative team.
  • Flexibility to travel to skilled nursing facilities within the designated geographic area.
