Job Description:

A care coordinator plays a crucial role in coordinating and managing the care of individuals, ensuring they receive the necessary services and support. The job duties of a care coordinator may include:
  • Conducting initial assessments to determine clients' healthcare needs, including physical, mental, and social needs.
  •  50% field and 50% at home to document and follow up with members over the phone
  • Location you would be serving Coral Gables, Miami Beach, North Miami, Little Havana, Downtown Miami.
  •  Collaborating with clients, healthcare providers, and other professionals to create individualized care plans that address clients' specific needs and goals.
  • Serving as a central point of contact for clients and their healthcare team, ensuring that all necessary services and resources are accessed and coordinated effectively.
  • Assisting clients in navigating the healthcare system, advocating for their rights, and ensuring they receive appropriate and timely care.
  •  Regularly reviewing clients' progress and adjusting care plans as needed, ensuring that goals are being met and that clients are receiving the necessary support
  • Offering education and support to clients and their families, helping them understand their condition, treatment options, and available resources.
  •  Coordinating referrals to specialists, diagnostic tests, and other healthcare services, ensuring timely and appropriate access to care.

Requirements 
  • Must be bilingual Spanish/English 
  • Must have reliable transportation and valid drivers license 
  • Must have 3 years of care coordination 
  • Bachelors degree preferred
