Remote Care Manager (Bilingual) - RN/LMSW/LCSW

Job Title: Remote Care Manager

Location: 100% Remote (United States), but must be licensed in New York State. Candidates not residing in the tri-state area will not be eligible for permanent conversion.

Job Description: We are seeking a Remote Care Manager to join our team, responsible for managing a caseload of adult and geriatric patients enrolled in Managed Long-Term Care (MLTC) programs in New York State. The ideal candidate will have a strong understanding of MLTC and NY Medicaid policy, with experience in creating and implementing Person-Centered Care Plans for patients. This role requires proficiency in conducting outreach calls, conducting Person-Centered Care Plan reviews, and coordinating care services for patients.

Key Responsibilities:

  • Conduct outreach calls to patients to assess their needs and preferences.
  • Review and update Person-Centered Care Plans in collaboration with patients, caregivers, and healthcare providers.
  • Coordinate care services and resources to ensure patients receive appropriate support.
  • Manage a caseload of 225-230 patients, with a focus on completing 2-3 Person-Centered Service Plan (PCSP) reviews per day.
  • Fluent in Spanish, Russian, Bengali, Mandarin, Cantonese, or other languages as needed to communicate effectively with patients.
  • Maintain accurate documentation of patient interactions and care plans in compliance with regulatory standards.
  • Collaborate with interdisciplinary teams to address patients' medical, social, and behavioral health needs.

Requirements:

  • RN/LMSW/LCSW licensure in the State of New York.
  • Previous experience working in MLTC and knowledge of UAS Assessment.
  • Strong communication and interpersonal skills.
  • Ability to work independently and remotely, with a minimum of 7 successful calls per day and 125 member contacts per month.
  • Tech-savvy with experience working in remote settings.

Preferred Qualifications:

  • Experience in a remote care management role.
  • Familiarity with telehealth platforms and electronic health records (EHR) systems.
  • Ability to adapt to changing priorities and manage time effectively.
