Job Summary: The Bilingual Community Outreach Coordinator plays a vital role in supporting members, providers, and multidisciplinary team members to assess, plan, and coordinate integrated care delivery across the continuum, focusing on behavioral health and long-term care for members with high potential needs. This position is responsible for ensuring that patients progress towards desired outcomes with quality, medically appropriate, and cost-effective care based on the severity of illness and site of service. The ideal candidate is bilingual, compassionate, and has a strong commitment to serving underserved populations.

Essential Position Responsibilities:

  • Serve as a community-based member advocate and resource, leveraging knowledge of the community and available resources to engage and assist vulnerable members in managing their healthcare needs.
  • Collaborate with and support the Healthcare Services team by providing non-clinical paraprofessional duties in the field, including meeting with members in their homes, nursing homes, shelters, or doctor's offices.
  • Empower members by helping them navigate and maximize their health plan benefits, including scheduling appointments with providers, arranging transportation for healthcare visits, and following up with members on missed appointments.
  • Assist members in accessing social services such as community-based resources for housing, food, and employment.
  • Provide outreach to locate and provide support for disconnected members with special needs.
  • Conduct research with available data to locate members the company has been unable to contact, including reviewing internal databases, contacting member providers or caregivers, or traveling to last known addresses or community resource locations.
  • Participate in ongoing or project-based activities that may require extensive member outreach, both telephonically and face-to-face.
  • Guide members to maintain Medicaid eligibility and assist with accessing other financial resources as appropriate.
  • 50-80% local travel may be required. Reliable transportation is required.

Position Qualifications:

  • High School Diploma/GED required.
  • Minimum 1 year of experience working with underserved or special needs populations, with varied health, economic, and educational circumstances.
  • Must have a valid driver's license with a good driving record and be able to drive within applicable state or locality with reliable transportation.

Preferred Qualifications:

  • Bilingual proficiency in Spanish or Bengali
  • Experience working in healthcare or social services settings.
  • Knowledge of Medicaid and other public health programs.
  • Strong interpersonal and communication skills.
