Salary: $80,000-$95,000 + benefits

About the Role:
Join a leading national managed care organization as an RN Care Coordinator, where you will play a vital role in ensuring that patients receive seamless, high-quality healthcare services. This position focuses on coordinating care across the healthcare continuum, developing individualized care plans, and acting as a central point of contact between patients, families, and healthcare providers. You’ll leverage your clinical expertise and organizational skills to support patient outcomes in a compassionate, patient-centered environment.

Key Responsibilities:

  • Coordinate and oversee patient care across multiple healthcare services to ensure a cohesive approach to treatment.
  • Develop personalized care plans based on comprehensive patient assessments.
  • Facilitate effective communication among patients, families, and healthcare teams to enhance care continuity.
  • Track patient progress, maintain accurate records, and ensure compliance with healthcare standards.
  • Educate patients and their families about care options, medications, and treatment plans to promote informed decisions.
  • Schedule appointments, referrals, and follow-ups to support timely and efficient care delivery.
  • Collaborate on quality improvement initiatives to enhance both patient care and operational processes.

Qualifications:

  • Bachelor’s degree in nursing, public health, or a related healthcare field.
  • Minimum of 2 years of clinical experience as a Registered Nurse in a home health or similar setting.
  • Active Nevada RN license (required).
  • Demonstrated expertise in care coordination, case management, or a similar role.
  • Strong communication and interpersonal skills to effectively engage with diverse patients and healthcare professionals.
  • Experience working collaboratively within multidisciplinary teams.
  • Proficiency in electronic health record (EHR) systems and Microsoft Office Suite.
  • Exceptional organizational and time management skills to handle multiple priorities in a fast-paced environment.
