Key Responsibilities:
- Coordinate and oversee patient care across multiple healthcare services to ensure a cohesive approach to treatment.
- Develop personalized care plans based on comprehensive patient assessments.
- Facilitate effective communication among patients, families, and healthcare teams to enhance care continuity.
- Track patient progress, maintain accurate records, and ensure compliance with healthcare standards.

Qualifications:
- Bachelor’s degree in nursing, public health, or a related healthcare field.
- Minimum of 2 years of clinical experience as a Registered Nurse in a home health or similar setting.
- Active Nevada RN license (required).
- Demonstrated expertise in care coordination, case management, or a similar role.
- Strong communication and interpersonal skills to effectively engage with diverse patients and healthcare professionals.
- Experience working collaboratively within multidisciplinary teams.
- Proficiency in electronic health record (EHR) systems and Microsoft Office Suite.
- Exceptional organizational and time management skills to handle multiple priorities in a fast-paced environment.
