Pay: $48.00 per hour + Full benefits

Position Summary:

We are seeking a compassionate, organized, and detail-oriented Care Coordinator to join our team. This role acts as a critical liaison between home care providers, home health agencies, and insurance companies to ensure that clients receive high-quality, coordinated, and cost-effective care. The Care Coordinator will advocate for clients and help navigate the complex healthcare system, ensuring services are delivered seamlessly and in alignment with individualized care plans.


Key Responsibilities:

  • Serve as the central point of contact for clients, families, care providers, and insurance representatives.

  • Coordinate and monitor the delivery of home care and home health services, ensuring continuity and quality of care.

  • Collaborate with interdisciplinary teams (nurses, social workers, therapists, etc.) to develop and update care plans based on client needs and goals.

  • Review insurance authorizations, verify eligibility and coverage, and help resolve billing or service-related issues.

  • Ensure documentation and communication meet compliance, regulatory, and organizational standards.

  • Track client progress, assess satisfaction, and facilitate adjustments in services as needed.

  • Educate clients and families on available services, resources, and best practices for care at home.

  • Maintain accurate records in care management systems and contribute to ongoing quality improvement initiatives.


Qualifications:

  • Bachelor's degree in Health Administration, or a related field (or equivalent experience).

  • 2+ years of experience in care coordination, home care/home health, or case management.

  • Knowledge of insurance processes, including Medicaid/Medicare and managed care organizations.

  • Excellent communication, problem-solving, and organizational skills.

  • Ability to work independently and collaboratively across teams.

  • Proficient in electronic health records (EHR) and Microsoft Office Suite.

  • A valid RN or LSW license is a plus
