Social Worker / LCSW
Location: Eureka, CA
Sign-On Bonus: $2,500 (paid after 90 days of employment, no payback required)

Position Overview:
We are seeking a compassionate and experienced Social Worker to join our interdisciplinary care team serving frail and elderly adults in a community-based setting. Reporting to the Social Work Manager, this role is responsible for delivering psychosocial assessments, care planning, counseling, case management, and ensuring smooth transitions of care for our participants. The ideal candidate will have experience with aging populations, care coordination, and working in a PACE, hospice, or home health setting.

Key Responsibilities:

  • Conduct psychosocial assessments to determine participants’ needs, preferences, and goals

  • Participate in interdisciplinary team (IDT) meetings to develop and update individualized care plans

  • Deliver and document social work services in alignment with care plans, including case management and individual/group counseling

  • Coordinate care transitions between hospitals, skilled nursing facilities, and home settings

  • Act as a liaison between participants, families, and the care team

  • Facilitate care conference meetings involving participants and their support networks

  • Provide discharge planning for participants who disenroll from the program

  • Assist participants with behavioral health and substance use concerns as needed

Qualifications:

  • Master’s degree in Social Work (MSW) required

  • Licensed Clinical Social Worker (LCSW) in California preferred; ASW considered with relevant experience

  • Minimum of one year of experience working with frail or elderly populations

  • Strong skills in care coordination, crisis intervention, and psychosocial support

  • Experience with interdisciplinary teams and care planning in a medical or community-based setting

  • Familiarity with Medicare, Medi-Cal, and community resource navigation
