Morgan Stephens, a national healthcare recruiting firm, is seeking experienced Licensed Practical Nurses (LPNs) for several direct hire roles with respected healthcare and home care organizations. These openings span care coordination, utilization review, and field-based case management. Open Positions
LPN – Home Care (Full-Time or Per Diem) - Nashua, Londonderry, Manchester
Provide compassionate in-home nursing care to clients in southern New Hampshire. This role offers flexible scheduling for per diem nurses and exceptional benefits for full-time employees.
Deliver direct patient care including medication administration, wound care, injections, and monitoring vital signs
Support chronic disease management and provide patient and family education
Accurately document all care in the agency’s electronic medical record
Communicate changes in patient condition to clinical supervisors and physicians
Travel locally to patient homes; mileage reimbursement provided
Work independently while maintaining high standards of care and professionalism
Case Manager II – Hybrid (Nashua and surrounding areas)
Coordinate long-term services and supports (LTSS) for Medicaid members
Complete clinical assessments and develop person-centered care plans
Conduct two weekly in-person visits to skilled nursing facilities and manage remote casework from home
Collaborate with interdisciplinary teams and connect members to community-based resources
Position includes mileage reimbursement for field visits
Utilization Review Nurse – Remote
Review clinical services for medical necessity based on established guidelines
Ensure quality and cost-effective care through provider collaboration
Requires strong documentation and communication skills
Fully remote with flexible hours
Case Manager II LTSS – Remote
Support Medicaid members through assessment, planning, and care coordination
Ensure services are delivered in the least restrictive setting
Maintain documentation in accordance with Medicaid requirements
Collaborate with behavioral health, social services, and long-term care providers
Qualifications for All Roles
Active LPN license in the state of New Hampshire
Minimum one to three years of clinical nursing experience
Five or more years preferred for coordinator roles with supervisory experience
Home health or managed care experience preferred
Proficiency with EMR systems and documentation
CPR and First Aid certification
Reliable transportation and valid driver’s license for field-based roles
Home office setup with high-speed internet for remote roles
