Salary: $110,000 – $120,000 base plus commission

Key Responsibilities:
Build and nurture strong referral relationships with hospitals, primary care providers, discharge planners, skilled nursing facilities, and senior living communities.
Represent our home care services in the community through scheduled sales visits, networking events, and professional presentations.
Execute a data-driven, strategic sales plan to achieve and exceed monthly admissions and referral targets.
Act as a liaison between referral sources and internal clinical teams to ensure seamless patient onboarding and service satisfaction.
Qualifications:
Prior success in healthcare sales, ideally in home care, home health, or hospice.
Deep understanding of the local healthcare market in Suffolk County and surrounding areas.
Strong communication, negotiation, and follow-through skills.
Independent, highly motivated, and organized with the ability to work in the field.
Valid NY driver's license and dependable transportation required.
Preferred:
Existing book of referral sources is strongly preferred.
Clinical background a plus, but not required.
