Location: Manchester / Concord, NH Area (including Epping and surrounding towns)
Employment Type: Per Diem, FT or PT

As an Admissions Licensed Practical Nurse (LPN) with Guardian Home Care of NH, you will play a vital role in welcoming new clients into care by conducting comprehensive admission visits and ensuring each client’s transition into home care is smooth, compliant, and compassionate. You will be the first clinical point of contact for new clients—verifying eligibility, gathering clinical data, and helping establish an individualized Plan of Care.

You will work under the supervision of the Clinical Director, Director of Operations, or Registered Nurse assigned to the case, ensuring all admissions are performed in accordance with state regulations, agency policy, and established best practices.


Compensation

This is a per diem position with flexible scheduling.
Hourly pay ranges from $34 to $36 per hour
Visit rate: 1.5x the hourly rate (for example, $35/hr = $52.50 per visit)
Admission rate: 2x the hourly rate (for example, $35/hr = $70.00 per admission)
Assignments are primarily within the Manchester and Concord territories, with occasional coverage opportunities extending toward Epping and nearby communities (approximately 30–45 minutes travel radius).


Educational and Professional Requirements

Graduate of an accredited school of nursing; current license as a Licensed Practical Nurse in the State of New Hampshire (required).
Minimum of one (1) year of nursing experience, preferably in home health, hospice, or community care settings.
Knowledge of Medicare/Medicaid guidelines and home care admission procedures (preferred).
Current CPR certification.
Reliable transportation and willingness to travel to client homes.


Core Responsibilities

Conduct initial admission visits for new home care clients, including completion of admission forms, medication reconciliation, and baseline assessments.
Perform comprehensive evaluations of the client’s physical, emotional, and environmental needs.
Verify eligibility and consent documentation, ensuring that all required forms are completed accurately prior to initiating services.
Collaborate with the RN Case Manager, Clinical Director, and physician to establish and initiate the Plan of Care based on clinical findings and client needs.
Review medical histories, medications, and physician orders, identifying inconsistencies or contraindications that require follow-up.
Provide education to clients and families regarding care expectations, agency policies, safety, and emergency procedures.
Ensure timely entry of admission documentation into the EMR system and communicate status updates to the clinical and scheduling teams.
Identify and report any immediate client needs or potential safety concerns to the RN and/or Director of Operations.
Coordinate with scheduling and intake teams to ensure a seamless start-of-care transition following the admission visit.
Participate in team meetings, staff education, and quality improvement activities as assigned.

Proficiency in EMR systems and ability to complete digital documentation in real-time.
