Position Summary
A leading managed care organization serving Kansas Medicaid members is seeking a compassionate and detail-oriented Certified Nursing Assistant (CNA) to join its Care Management team. This is a hybrid telephonic and field-based role designed to support members with chronic and complex health needs by helping them access home- and community-based services while assisting nurses and care managers with coordination and follow-up activities.
The Care Coordinator conducts outreach calls, gathers health and social information, assists with appointment and service coordination, and provides ongoing follow-up support to members and families. Occasional in-person visits may be required for wellness checks, care coordination, or documentation support (mileage reimbursed).
This role is ideal for an experienced CNA or home health professional looking to transition into care coordination, case management support, or managed care operations while maintaining meaningful member and family interaction.
Key Responsibilities
• Conduct outbound and inbound telephonic outreach to members, caregivers, and providers to confirm appointments, review service needs, and support adherence to care plans
• Support RN and care managers by gathering member updates, verifying services and authorizations, and documenting interactions in the electronic health record
• Assist with scheduling and coordination of community-based services, including follow-up after hospital or skilled nursing facility discharges
• Perform limited in-home or community visits to assess member well-being, observe home safety concerns, or provide informational materials under RN supervision
• Identify barriers to care such as transportation, housing, or access to services and escalate concerns to nursing or social work staff
• Educate members and families on health plan benefits, covered services, wellness programs, and preventive care resources
• Maintain timely, accurate, and compliant documentation of all member interactions in accordance with HIPAA and organizational policies
• Collaborate closely with an interdisciplinary care team including nurses, social workers, and utilization or authorization staff to ensure coordinated, high-quality care
Required Qualifications
• Active Certified Nursing Assistant (CNA) certification
• Minimum 1 year of experience in home health, long-term care, or community-based healthcare
• Strong verbal communication skills with a professional, empathetic phone presence
• Comfort working in electronic documentation systems and basic proficiency with Microsoft Office tools
• Reliable transportation for occasional local field visits (mileage reimbursed)
• Strong organizational skills with the ability to manage multiple tasks in a hybrid or remote environment
Preferred Experience
• Previous experience in care coordination, member services, or managed care environments
• Familiarity with Kansas Medicaid and home- and community-based service programs
• Experience supporting members with chronic conditions such as diabetes, COPD, heart disease, or mobility limitations
• Background working with vulnerable or underserved populations
