About the Opportunity
We are seeking a relationship-driven and organized Home Care Coordination Specialist to support client onboarding, referral coordination, scheduling communication, and community outreach for a growing home care organization in New Hampshire.
This position plays a key role in ensuring smooth coordination between referral sources, clients, caregivers, scheduling teams, and clinical staff. The ideal candidate enjoys working in a fast-paced healthcare environment, communicating with community partners, and helping ensure clients receive timely and compassionate services.

Compensation and Benefits

What You’ll Be Doing
• Coordinate communication between referral sources, case managers, clients, caregivers, and internal departments
• Support intake, admissions, and client onboarding processes
• Assist with scheduling coordination and continuity of care communication
• Respond to client and referral inquiries professionally and compassionately
• Track referrals, pending documentation, and follow-up activities accurately
• Help identify urgent client needs, service barriers, or medically complex situations and communicate appropriately to leadership
• Maintain accurate documentation within HHAeXchange and internal systems
• Collaborate closely with scheduling, intake, hiring, operations, and clinical teams
• Build relationships with community organizations, referral partners, and case managers
• Attend community meetings, networking opportunities, and outreach events as needed
• Support office coordination, documentation, and operational workflows
Qualifications
• Minimum of 2 years of experience in home care, healthcare coordination, scheduling, intake, medical office support, or case management preferred
• Experience in home care or healthcare environments strongly preferred
• Familiarity with HHAeXchange or similar EMR systems preferred
• Strong communication, multitasking, and organizational skills
• Ability to work independently and collaboratively in a fast-paced office environment
• Professional, outgoing, and relationship-oriented personality
• Basic medical or clinical understanding preferred
• Proficiency with Microsoft Office and Teams
• High attention to detail and documentation accuracy
