Scheduler / Scheduling Coordinator – Home Care (In-Office)

Compensation: $30–$34 per hour

Overview

We are seeking a dedicated and highly organized Scheduler / Scheduling Coordinator to join our growing Home Care team. In this vital role, you will coordinate caregiver schedules, support client service delivery, and help ensure continuity of care for individuals receiving home and community-based services.

The ideal candidate will have experience in home care scheduling, Medicaid-funded programs, LTSS (Long-Term Services and Supports), waiver services, or care coordination environments. Candidates with experience obtaining Medicaid authorizations, coordinating services under managed Medicaid plans, or working with Medicaid-based home care programs are strongly encouraged to apply.

Responsibilities

• Develop and maintain accurate daily, weekly, and monthly schedules for caregivers and home care staff based on client needs and employee availability

• Coordinate caregiver assignments based on client care requirements, geographic coverage, skills, certifications, and schedule preferences

• Communicate effectively with clients, caregivers, case managers, and internal teams to confirm schedules and resolve conflicts

• Adjust schedules proactively in response to call-outs, client changes, emergencies, hospitalizations, or service interruptions

• Maintain detailed records of caregiver assignments, client preferences, visit schedules, and service authorizations within scheduling systems

• Monitor service delivery to ensure compliance with client care plans, authorized service hours, and agency standards

• Collaborate with intake, care coordination, operations, and clinical teams to support continuity of care

• Assist with tracking Medicaid authorizations, service approvals, and authorized hours to ensure services are delivered appropriately

• Coordinate services for clients receiving Long-Term Services and Supports (LTSS), waiver services, personal care services, or other Medicaid-funded programs

• Work closely with case managers, managed care organizations, and referral sources regarding service changes and authorization updates

• Generate reports on scheduling activity, caregiver utilization, open shifts, service delivery metrics, and client coverage needs

• Support agency growth by identifying staffing needs and helping optimize caregiver utilization

Qualifications

• Minimum of 2 years of scheduling, staffing coordination, or workforce management experience in home care, healthcare, LTSS, Medicaid, or related settings

• Experience with home care scheduling strongly preferred

• Experience working with Medicaid-funded services, LTSS programs, waiver programs, personal care services, or managed Medicaid populations preferred

• Experience reviewing, tracking, or coordinating Medicaid authorizations strongly preferred

• Knowledge of home care operations, caregiver staffing, and service delivery models preferred

• Strong organizational skills with the ability to prioritize multiple responsibilities in a fast-paced environment

• Excellent verbal and written communication skills

• Proficiency with scheduling software, EMR systems, and Microsoft Office Suite

• Experience with HHAeXchange, Access Care, ClearCare, AxisCare, AlayaCare, or similar platforms is a plus

• Ability to solve problems quickly and adapt to changing circumstances

• Strong attention to detail and accuracy in documentation and recordkeeping

• Knowledge of Medicaid regulations, home care compliance requirements, and community-based service programs is highly desirable
