About the Opportunity

We are seeking a compassionate, organized, and relationship-driven Care Coordination Specialist – Admissions & Intake to support client admissions, intake coordination, referral management, and continuity of care for a growing home care organization in New Hampshire.

This position requires experience in home care or working with home care agencies.

This position serves as a critical link between referral sources, clients, families, caregivers, scheduling teams, and clinical staff. The ideal candidate enjoys helping clients navigate the admissions process, coordinating services, communicating with healthcare professionals, and ensuring clients receive timely and compassionate care.

What You'll Be Doing

• Coordinate client admissions, intake, and onboarding processes

• Serve as a primary point of contact for referral sources, case managers, clients, families, caregivers, and healthcare professionals

• Assist clients and families through the start-of-care process, ensuring a positive and supportive experience

• Coordinate communication between referral sources, scheduling teams, caregivers, clinical staff, and operations

• Review referrals, intake documentation, service requests, and care coordination needs

• Track referrals, admissions, pending documentation, authorizations, and follow-up activities

• Assist with scheduling coordination and continuity of care communication

• Help identify urgent client needs, service barriers, or medically complex situations and communicate appropriately to leadership

• Maintain accurate documentation within HHAeXchange and internal systems

• Collaborate closely with scheduling, recruiting, operations, intake, and clinical teams

• Build relationships with hospitals, rehabilitation facilities, physician offices, case managers, discharge planners, managed care organizations, and community partners

• Participate in community outreach, networking opportunities, and referral relationship development as needed

• Support office coordination, documentation, reporting, and operational workflows

Qualifications

• Minimum of 2 years of experience in home care, healthcare coordination, admissions, intake, scheduling, medical office support, case management, care coordination, or healthcare administration preferred

• Experience in home care, home health, hospice, healthcare, or community-based services strongly preferred

• Experience working with referrals, admissions, client onboarding, or care coordination processes preferred

• Familiarity with HHAeXchange or similar EMR systems preferred

• Strong communication, multitasking, customer service, and organizational skills

• Ability to work independently and collaboratively in a fast-paced office environment

• Professional, compassionate, and relationship-oriented personality

• Basic medical terminology and healthcare knowledge preferred

• Proficiency with Microsoft Office and Teams

• High attention to detail and documentation accuracy
