We are seeking an experienced and compassionate Registered Nurse (RN) to serve as a Home Care Charge Nurse supporting a growing home care team throughout the Wichita area. This leadership position combines clinical oversight with hands-on patient care, making it an excellent opportunity for an RN who enjoys mentoring field staff while remaining engaged in direct patient care.
The Charge Nurse will provide clinical leadership for field nurses, ensure quality standards are maintained, and periodically conduct skilled nursing visits and patient assessments as needed.
The RN Home Care Charge Nurse serves as a clinical resource for the nursing team while helping coordinate high-quality care for patients receiving services in their homes. This position works closely with office leadership, caregivers, therapists, physicians, and families to promote excellent patient outcomes and regulatory compliance.
• Provide day-to-day clinical supervision and support for field nurses.
• Serve as a clinical resource for patient care questions, documentation, and nursing best practices.
• Perform patient assessments, admissions, supervisory visits, and skilled nursing visits as needed.
• Review and monitor patient care plans to ensure quality and regulatory compliance.
• Collaborate with physicians and interdisciplinary care teams regarding changes in patient condition.
• Assist with scheduling, staffing coordination, and clinical problem-solving when needed.
• Participate in orientation, mentoring, and ongoing education of nursing staff.
• Ensure timely and accurate clinical documentation within the electronic medical record.
• Promote excellent patient satisfaction, quality outcomes, and compliance with agency policies and state regulations.

• Current Registered Nurse (RN) license in the State of Kansas.
• Minimum of two years of nursing experience, with home health or community-based care preferred.
• Previous supervisory, charge nurse, or clinical leadership experience strongly preferred.
• Strong assessment, clinical decision-making, and communication skills.
• Experience coordinating care with physicians, families, and interdisciplinary healthcare teams.
• Valid driver's license and reliable transportation for local field visits.
