Salary: $75,000–$80,000 annually, based on experience and qualifications
We are seeking an experienced and compassionate Registered Nurse (RN) to join our growing home care team serving the Wichita area. This full-time position is ideal for an RN who enjoys building meaningful relationships with patients while providing high-quality skilled nursing care in the comfort of their homes.
The Registered Nurse will manage a caseload of home care patients, complete comprehensive assessments, develop individualized care plans, coordinate services with interdisciplinary team members, and promote positive patient outcomes through compassionate, evidence-based care.
The Home Care RN is responsible for providing skilled nursing services, supervising patient care, and ensuring compliance with physician orders, state regulations, and agency standards. This position offers autonomy in the field while working closely with office leadership, caregivers, therapists, physicians, and families to deliver exceptional patient-centered care.
• Complete comprehensive patient assessments and admissions
• Develop, implement, and update individualized plans of care
• Provide skilled nursing visits in accordance with physician orders
• Monitor patient progress and identify changes in condition
• Coordinate care with physicians, therapists, caregivers, and other healthcare professionals
• Provide patient and caregiver education regarding medications, disease management, and home safety
• Complete supervisory visits for caregivers as required
• Maintain timely and accurate documentation within the electronic medical record
• Participate in care conferences and interdisciplinary team meetings
• Ensure compliance with agency policies, Medicare and Medicaid guidelines, and state regulations

• Current Registered Nurse (RN) license in the State of Kansas
• Minimum of two years of nursing experience
• Home health, home care, hospice, or community nursing experience preferred
• Strong assessment, clinical decision-making, and communication skills
• Experience developing care plans and coordinating interdisciplinary care
• Ability to work independently while managing a patient caseload
• Valid driver's license and reliable transportation for local travel
