We are seeking a reliable and experienced Registered Nurse (RN) to provide in-home assessments and support clients throughout Harford and Cecil Counties, Maryland. This role is ideal for nurses looking for flexible, supplemental income while making a meaningful impact in patient care and regulatory compliance.

This position primarily supports required client assessments and care coordination in a home setting. Travel between client homes is required.

Service Area

Primary coverage includes Havre de Grace, Aberdeen, Bel Air, Edgewood, Abingdon, Joppa, Fallston, Churchville, Perryville, North East, Elkton, Rising Sun, Chesapeake City, Port Deposit, and surrounding communities throughout Harford and Cecil Counties.

Responsibilities

• Conduct in-home patient assessments and develop individualized care plans

• Monitor patient condition and document findings accurately

• Ensure compliance with Maryland regulations and assessment timelines

• Communicate changes in patient condition to physicians and appropriate care team members

• Coordinate care with physicians, caregivers, and other healthcare professionals

• Educate patients and families regarding care plans and health conditions

• Maintain accurate, timely, and compliant documentation

Qualifications

• Active Registered Nurse (RN) license in the State of Maryland

• Home care or home health experience preferred

• Ability to work independently and efficiently manage a flexible schedule

• Strong communication, assessment, and documentation skills

• Reliable transportation and willingness to travel throughout Harford and Cecil Counties
