Compensation: $67,000 – $72,000 per year + Full Benefits

Position Summary
The Resident Services Coordinator plays a key role in supporting the Social Services Department and ensuring residents receive high-quality, individualized care. This position focuses on care coordination, resident advocacy, and supporting families through the complexities of long-term care and aging services.
Key Responsibilities

Assist in the overall management and daily operations of the Social Services Department

Support and guide a multidisciplinary care team to deliver resident-centered services

Provide emotional, social, and community-based support to residents and their families

Conduct resident assessments and coordinate case management services

Assist residents and families in navigating long-term care resources, benefits, and entitlements

Maintain accurate, timely, and compliant documentation of services and interactions

Promote and uphold quality of life standards in alignment with regulatory and organizational guidelines
Qualifications

Master’s degree in Social Work (MSW) required

NY State LMSW (or working toward licensure) preferred

Minimum of 2 years of experience in geriatrics, long-term care, or case management preferred

Experience with discharge planning and assessing the needs of an aging population preferred

Background in healthcare, aging services, or long-term care settings a plus

Strong communication, organizational, and documentation skills

Ability to collaborate effectively with interdisciplinary teams
