About the Opportunity
Guardian Home Care is seeking a Nurse Practitioner for a contract / PRN opportunity supporting our Michigan home care services.
The primary focus is completing clinical assessments, medical needs paperwork, and clinical packets, including completing and signing MDHHS-6200 Adult Services Medical Needs Certification forms within your professional scope and applicable program requirements.
This opportunity may be a good fit for an experienced Nurse Practitioner seeking supplemental work on an as-needed basis.
Responsibilities

Complete clinical assessments and evaluate clients’ medical and personal care needs.

Complete and sign MDHHS-6200 medical needs certification forms when clinically appropriate and supported by the assessment and available records.

Prepare complete, accurate clinical packets and supporting documentation.

Review medical information and identify additional records or clinical details needed to complete paperwork.

Coordinate with Guardian’s clinical and administrative teams to support timely submission of required documentation.

Maintain client confidentiality and follow applicable professional and program standards.
Required Qualifications

Active Michigan Registered Nurse (RN) license and Michigan Nurse Practitioner specialty certification.

Master’s degree or higher in nursing.

Current national Nurse Practitioner certification.

Active individual National Provider Identifier (NPI) number.

Strong clinical assessment, documentation, and organizational skills.

Ability to work independently and communicate effectively with the agency team.

Current Michigan Medicaid enrollment through CHAMPS, or willingness to complete the appropriate enrollment before performing services that require it. Assistance is available.
Preferred Qualifications

Experience completing medical needs certifications, clinical assessments, or similar documentation.

Experience with home care, community-based care, or Medicaid-funded programs.

Familiarity with Michigan’s Home Help program and MDHHS-6200 forms.
