Job Description: We are seeking a compassionate and dedicated Case Manager to join our team. The ideal candidate will have a Bachelor's degree in Health and Human Services/Science, Psychology, Sociology, Criminology/Criminal Justice, or a related field from a US-based institution. This role requires a minimum of 1 year of experience working with at-risk clients in a direct service setting.

Responsibilities:

  • Provide direct support and assistance to individuals experiencing homelessness who reside in the shelter.
  • Conduct intake assessments and develop individualized service plans for clients.
  • Offer emotional support, guidance, and crisis intervention to clients in need.
  • Assist clients in accessing resources such as housing assistance, healthcare services, and job training programs.
  • Collaborate with other staff members and community partners to coordinate services and ensure comprehensive care for clients.
  • Maintain accurate and up-to-date client records and documentation.
  • Participate in staff meetings, trainings, and professional development activities as required.
  • Adhere to all organizational policies, procedures, and ethical standards.

Qualifications:

  • Bachelor's degree in Health and Human Services/Science, Psychology, Sociology, Criminology/Criminal Justice, or a related field.
  • Minimum of 1 year of experience working with at-risk clients in a direct service setting.
  • Knowledge of addiction and mental health issues is preferred.
  • Strong interpersonal and communication skills.
  • Ability to work effectively with individuals from diverse backgrounds.
  • Empathy, patience, and a nonjudgmental attitude toward clients.
  • Ability to maintain confidentiality and professionalism at all times.
