Home Care Aide - PCA / CNA - Full Remote (Macomb County Residents only)

Job Title: Remote Care Coordinator / Home Care Aide / PCA / CNA needed for full-time temp to perm position.

Pay: $19.00 per hour

Job Overview: We are seeking a compassionate and organized Remote Care Coordinator to support our clients in managing their care plans from the comfort of their homes. The ideal candidate will use their experience in patient care and strong communication skills to provide exceptional support to both clients and healthcare providers. This role is remote, offering the flexibility to work from home while making a meaningful impact on our clients' well-being.

Key Responsibilities:

  • Coordinate Care Plans: Collaborate with clients, families, and healthcare professionals to develop and maintain comprehensive care plans tailored to each client's needs.
  • Schedule Appointments: Arrange and manage appointments for medical visits, therapy sessions, and other essential services, ensuring seamless communication between clients and providers.
  • Client Communication: Act as the main point of contact for clients, providing information, answering questions, and offering support to address any concerns about their care.
  • Monitor Client Progress: Regularly check in with clients to track their progress, identify any changes in their needs, and adjust care plans as necessary.
  • Document Management: Maintain accurate, up-to-date client records and care documentation within electronic health record (EHR) systems, ensuring compliance with privacy regulations.
  • Facilitate Resource Access: Assist clients in accessing necessary healthcare resources, such as prescription refills, medical equipment, and community support services.
  • Support Caregivers and Family Members: Provide guidance and support to family members and caregivers, ensuring they understand the care plan and any specific needs of the client.
  • Administrative Tasks: Manage email correspondence, process documentation, and handle any other administrative tasks related to client care.

Qualifications:

  • Experience in Patient Care or Home Care: Previous experience as a Home Care Aide, CNA, or similar role is preferred.
  • Strong Communication Skills: Ability to clearly and empathetically communicate with clients, families, and healthcare professionals.
  • Organizational Skills: Highly organized with the ability to handle multiple client cases, appointments, and tasks simultaneously.
  • Proficiency in Technology: Comfortable using computers, scheduling software, and electronic health record (EHR) systems for managing client information.
  • Problem-Solving Skills: Ability to identify client needs, troubleshoot issues, and make timely decisions regarding care coordination.
  • Knowledge of Healthcare Services: Understanding of healthcare processes, terminology, and community resources to effectively support clients.
  • Adaptability and Empathy: Able to respond to changing client needs with patience, compassion, and a willingness to go the extra mile.

Preferred Skills:

  • Bilingual Abilities: Speaking multiple languages is a plus, particularly Spanish or other widely spoken languages.
  • Certification: Certification as a Nursing Assistant (CNA) or Home Health Aide (HHA) is a plus but not required.
