RN CASE MANAGER - CENTRAL VIRGINIA (FIELD & REMOTE) – MANAGED CARE ORGANIZATION
We are seeking a Registered Nurse (RN) Case Manager who resides in Central Virginia to join a dynamic Managed Care Organization. This role supports Medicaid members and requires pediatrics and maternity experience, as well as field-based travel to member homes for assessments.
Compensation: This is temp-to-perm paying $50.00 per hour
Location & Travel Requirements
- Remote position based in Central Virginia with required travel to member homes in Richmond, Ashland, Emporia, Farmville, Williamsburg, Hampton, and Newport News.
- Travel is 30-40% field-based with mileage reimbursement.
- High-speed internet and a home office setup required.
Position Overview
The RN Case Manager will work remotely and in the field, conducting face-to-face assessments and developing care plans for Medicaid members. The case manager will participate in interdisciplinary care team meetings and coordinate services to improve health outcomes.
Key Responsibilities
- Conducts clinical assessments and determines eligibility for case management based on health needs.
- Develops and implements individualized care plans in collaboration with the member, caregivers, and healthcare professionals.
- Performs telephonic and in-person visits to assess and monitor members’ health status and needs.
- Ensures continuity of care by integrating behavioral health services, long-term care support, and wellness programs (e.g., asthma and depression management).
- Facilitates interdisciplinary care team (ICT) meetings to coordinate services.
- Uses motivational interviewing techniques to educate and support members.
- Identifies and addresses barriers to care, providing resources to improve member outcomes.
- Maintains an ongoing caseload, ensuring compliance with case management requirements.
Schedule & Work Expectations
- Monday through Friday, 8:00 AM - 5:00 PM (No weekends).
- Travel required 30%+ for face-to-face assessments.
- Caseload: 175-200 members.
Required Qualifications
- Active, unrestricted RN license in Virginia.
- 1-3 years of case management, disease management, managed care, or healthcare experience.
- Background in pediatrics and/or maternity nursing.
- Strong computer skills for multitasking between systems, documenting interactions, and managing case files.
- Must possess a valid driver’s license with a clean driving record and reliable transportation for field visits.
- Bachelor’s Degree in Nursing (BSN) preferred.
Preferred Qualifications
- Experience working in Managed Care Organizations (MCOs).
- Familiarity with Medicaid programs and community-based services.
- Experience with behavioral health case management is a plus.
