Pay: $40-$43/hour
Assignment Type: Temp-to-Perm
Start Date: As soon as available
Work Schedule: Monday – Friday, 8:00 AM – 5:00 PM EST (No weekends)
Tax Work Location: Hybrid

Position Overview
The Case Manager II will support Medicaid-eligible members receiving Long Term Services and Supports (LTSS) in southern New Hampshire, primarily in the Nashua area. This hybrid role includes two days per week of field-based visits to skilled nursing facilities (SNFs) and three days working remotely from home performing telephonic case management. You will assess member needs, develop individualized care plans, and coordinate services across medical, behavioral, and social domains to enhance health outcomes and quality of life.


Key Responsibilities

  • Complete initial and ongoing clinical assessments of members in accordance with New Hampshire Medicaid timelines and requirements

  • Develop person-centered, individualized care plans in collaboration with members, families, providers, and facility staff

  • Conduct face-to-face visits in SNFs twice weekly and maintain telephonic contact with members as needed

  • Monitor care plans for effectiveness, compliance, and goal progression; revise plans as member needs evolve

  • Support integration of long-term care and behavioral health services, including referrals and resource navigation

  • Participate in interdisciplinary care team (ICT) meetings and collaborate with internal teams, including RN case managers and social workers

  • Utilize motivational interviewing techniques to engage members and encourage wellness and independence

  • Document all member interactions, assessments, and care plan activities in compliance with Medicaid standards

  • Identify and address social determinants of health and connect members with community-based services

  • Maintain regular communication and caseload management for assigned members

  • Travel locally within the Nashua area and surrounding southern NH counties (mileage reimbursement provided)


Required Qualifications

  • Must reside in New Hampshire, preferably near Nashua

  • 1–3 years of case management experience in managed care, long-term care, behavioral health, or public health

  • Valid driver’s license, reliable transportation, and good driving record

  • Proficiency with electronic documentation systems and strong multitasking skills

  • Home office setup with high-speed internet for remote work


Required Education / Licensure
One of the following is required:

  • Completion of an accredited LPN or LVN program (active NH license in good standing)

  • Bachelor’s or Master’s degree in Social Work, Nursing, Public Health, Psychology, Gerontology, or related field

  • Graduation from an accredited School of Nursing (BSN preferred)
