Salary: $90,000 range +
Sign-on: $2,500

We are seeking an exceptional and experienced Registered Nurse Case Manager to join our growing home health team in Salina, KS. Key Responsibilities:

  • Deliver skilled nursing visits to patients in their homes in accordance with the established plan of care

  • Coordinate care across the interdisciplinary team, patients, families, and physicians

  • Educate patients and families on disease process, medication, and care techniques to support recovery and independence

  • Perform and coordinate both in-person and telehealth visits as clinically appropriate

  • Complete patient admissions and develop individualized plans of care as needed

  • Train patients, caregivers, and staff on medication usage, durable medical equipment, and supplies

  • Participate in on-call rotation and provide clinical support as needed

  • Maintain accurate and timely documentation in compliance with regulatory requirements

Qualifications:

  • Degree in Nursing

  • Active RN license in the state of Kansas

  • Minimum of one year of home health experience (required)

  • Proficiency with electronic medical records (EMR) systems

  • Excellent communication, time management, and interpersonal skills

  • Strong critical thinking ability and a compassionate, patient-centered approach to care
