Location: Wichita, KS
Pay Rate: $27–$32 per hour
Schedule: Per Diem / As Needed
A leading home care organization in the Wichita area is seeking a compassionate and detail-oriented LPN to perform admissions, supervisory visits, and ongoing clinical check-ins for clients receiving in-home services. This per diem role is ideal for an LPN who values independence, flexibility, and the opportunity to make a meaningful impact on clients and caregivers.
The LPN will work in coordination with the Clinical Director to ensure care plans are followed, regulatory standards are met, and quality care is consistently delivered.
• Complete admission visits including assessments, documentation, and care plan initiation
• Conduct supervisory visits to evaluate caregiver performance and client satisfaction
• Monitor client condition and recommend updates to care plans as needed
• Provide caregiver coaching and reinforcement of care procedures
• Ensure compliance with agency policies, state regulations, and documentation requirements
• Maintain accurate clinical notes, assessments, and follow-up documentation
• Communicate with families, caregivers, and internal leadership regarding client needs
• Participate in quality reviews and case discussions when requested
Required
• Active LPN license in the State of Kansas (or compact license)
• Minimum of one year of experience in home care, home health, assisted living, or clinical nursing
• Strong assessment and documentation abilities
• Ability to work independently and manage field-based work efficiently
• Reliable transportation, valid driver’s license, and insurance
• Familiarity with electronic charting systems
Preferred
• Experience completing home care admissions or supervisory visits
• Understanding of home care regulatory and documentation practices
• Strong communication and mentoring skills
• $27–$32 per hour based on experience
• Additional mileage reimbursement may apply depending on visit type
