Salary Range: $65,000-$80,000 + 20% Bonus
Work Model: Remote (must reside in Connecticut; occasional local field meetings required)

About the Role

A growing national home care organization has recently launched operations in Connecticut and is seeking an experienced Medicaid Referrals / Outreach Manager to support expansion in the Stamford and surrounding markets. This role is responsible for building referral pipelines, managing waiver-related relationships, and driving client growth through Connecticut Medicaid programs.

The ideal candidate has hands-on experience with Connecticut HCBS waivers, understands the local case management ecosystem, and can work independently in a remote-first environment while maintaining strong local relationships.

Key Responsibilities

  • Develop and grow referral relationships tied to Connecticut Medicaid waiver programs

  • Work closely with case management organizations and community partners, including Area Agencies on Aging

  • Educate referral sources, clients, and families on home care services available under Medicaid waivers

  • Coordinate closely with internal intake, scheduling, and clinical teams to ensure smooth start-of-care

  • Track outreach efforts, referral activity, and conversion metrics

  • Attend occasional in-person meetings with referral partners in the local market

Required Waiver & Market Experience

  • CT HCBS for Elders Waiver

  • CT Personal Care Assistance (PCA) Waiver

  • Experience working with Connecticut case management organizations, including agencies such as Southwestern Connecticut Agency on Aging

Qualifications

  • 3+ years of experience working with Medicaid waiver programs in Connecticut

  • Strong understanding of CT DSS, HCBS workflows, and referral processes

  • Proven relationship-building and outreach experience

  • Ability to work remotely while managing a local market

  • Bachelor’s degree preferred or equivalent experience
