Case Manager - RN, LMSW, or LCSW (remote)

Job Summary:

Join our team as a Care Manager and make a meaningful impact on patient care. In this role, you'll oversee the transition of patients from hospital and short-term rehab settings back to their communities. Responsibilities include conducting disease management program education, ensuring continuity of care with primary care providers (PCP) and community resources, and completing care plans and assessments as needed. Some fieldwork may be required, and proficiency in Chinese a plus.

Key Responsibilities:

  • Manage the transition of patients from hospital or rehab to the community.
  • Provide disease management program education.
  • Maintain regular follow-up with patients to ensure continuity of care with PCP and community resources.
  • Complete care plans and assessments as indicated.
  • May involve some fieldwork.

Job Requirements:

  • 1-2 years of case management experience.
  • RN, LMSW, or LCSW license in New York.
  • Ability to work remotely.
  • Reside in New York, New Jersey, or Connecticut.
  • Availability during EST working hours (8:30 am - 5 pm).
  • Proficiency in case management and transition of care experience.

Summary:

As a Care Manager, you'll play a crucial role in managing the transition of care for our patients, ensuring optimal clinical, financial, and quality of life outcomes. Collaborating with multi-disciplinary teams, you'll proactively identify members in need of case management services, conduct assessments, and develop and implement care plans. You'll promote the integration of services to enhance continuity of care, attend regular meetings, and maintain professionalism while adhering to all documentation guidelines. With your strong communication and organizational skills, you'll make a significant difference in our patients' lives.

Required Education:

  • Bachelor's degree in Nursing, Social Work, or Health Education (experience and education combination considered).

Salary Range: $72,000-$101,000

Required Experience:

  • 5-7 years of clinical experience with case management experience.

Required Licensure/Certification:

  • Active, unrestricted State Registered Nursing license (RN) or Licensed Clinical Social Worker (LCSW) or Advanced Practice Social Worker (APSW) in good standing.
  • Valid driver's license with a good driving record for local travel.
