Overview:
We are seeking a compassionate and detail-oriented Clinical Care Coordinator to join our healthcare team. In this pivotal role, you will coordinate patient care activities to ensure the delivery of high-quality, patient-centered care throughout the healthcare continuum. Acting as a liaison between patients, families, and healthcare providers, you will facilitate effective communication and collaboration, assess patient needs, and develop care plans. Responsibilities:

  • Care Coordination:

    • Coordinate patient care across various disciplines to ensure optimal treatment outcomes.
    • Develop and implement individualized care plans based on patient assessments and needs.
    • Schedule appointments, referrals, and follow-up visits for patients.
  • Patient Advocacy and Education:

    • Act as a liaison among patients, families, and healthcare providers to ensure continuity of care.
    • Educate patients and families about treatment options, medications, and care procedures, empowering them to make informed decisions.
  • Monitoring and Documentation:

    • Monitor patient progress and maintain accurate, up-to-date records in compliance with healthcare regulations.
    • Participate in quality improvement initiatives to enhance patient care and operational efficiency.

Qualifications:

  • Education:

    • Bachelor’s degree in nursing, public health, or a related field.
  • Experience:

    • Minimum of 2 years as a Registered Nurse in a home health setting.
    • Proven experience in a clinical setting, preferably in care coordination or management.
  • Licensing/Certification:

    • Current nursing license (if applicable) or relevant certification in healthcare coordination.
  • Skills:

    • Strong communication and interpersonal skills to engage effectively with diverse patient populations.
    • Ability to work collaboratively in a multidisciplinary team environment.
    • Proficient in electronic health record (EHR) systems and Microsoft Office Suite.
    • Exceptional organizational and time management skills to handle multiple responsibilities and priorities.
