Position Overview:

We are seeking a dedicated and compassionate Registered Nurse (RN) to join our Home Health team in Saint Paul, MN. This remote, field-based role allows you to provide comprehensive nursing care to patients in their homes, ensuring the highest standards of patient-centered care. As a Home Health RN, you will assess patient health conditions, develop individualized care plans, administer treatments, and educate patients and families on managing health conditions.

Key Responsibilities:

  • Patient Assessments: Conduct thorough in-home patient assessments to determine healthcare needs and develop tailored care plans.
  • Care Coordination: Collaborate with physicians, therapists, and other healthcare professionals to ensure comprehensive and coordinated patient care.
  • Medication Administration: Administer prescribed medications, IV therapy, wound care, and other treatments per physician orders.
  • Patient & Family Education: Provide guidance and support to patients and their families on disease prevention, symptom management, and health maintenance.
  • Monitoring & Documentation: Track patient progress, update care plans as needed, and maintain accurate medical records in compliance with state and federal regulations.
  • Regulatory Compliance: Ensure adherence to home health policies, procedures, and regulatory requirements, maintaining high-quality care standards.
  • Independent Work: Utilize time management and critical thinking skills to efficiently provide care and document services in a remote environment.

Qualifications:

  • Current Registered Nurse (RN) license in the state of Minnesota (required).
  • Home Health experience required, with a strong understanding of in-home patient care needs.
  • Strong assessment and clinical skills, with an emphasis on delivering patient-centered care.
  • Excellent communication and interpersonal skills to interact effectively with patients, families, and healthcare teams.
  • Ability to work independently in a remote setting, managing patient care and documentation efficiently.
