Join our team as the Case Manager in our Housing Stability Service! The Case Manager position is responsible for working with clients participating in Department programs including the Stable Homes Program (including those participating in the HOPWA and the Homeless Prevention Programs), the Homeless Services Program (including those participating in Rapid Rehousing and Permanent Supportive Housing Programs), and Programs for Women Veterans, coordinating/updating referral services, and providing input regarding assigned cases. Also responsible for maintaining client files in accordance with agency policies, inputting client information in a designated database(s), and gathering and reporting program metrics. The successful candidate will perform these duties in a satisfactory manner and in accordance with policies, procedures and core values.
You have an Associate Degree; or
You have two (2) years of experience in lieu of education
